Well Cain was petty, insecure, dishonest, self serving, needy... and murderous.
And Ishmael was a result of not trusting God, nor was he the one intended for the promise. It was man's lack of faith and interference that put the "first son" there. The original goal was always to choose the son God granted.
Should God have chosen to bless those two? A murderer and a child created from a lack of faith, in adultery that He never intended to bless to begin with, because his creation was not what God had planned?
It seems to me it was more about humanities interference with God's plan putting God in a position to choose the second, rather than his intention.
when Cain the firstborn man approached God God saw his heart and warned him about sin knocking and waiting at his door but Cain chose to follow sin and disregard Gods word.
“And the Lord said unto Cain, Why art thou wroth? and why is thy countenance fallen?

If thou doest well, shalt thou not be accepted? and if thou doest not well, sin lieth at the door. And unto thee shall be his desire, and thou shalt rule over him.

And Cain talked with Abel his brother: and it came to pass, when they were in the field, that Cain rose up against Abel his brother, and slew him.”
‭‭Genesis‬ ‭4:6-8‬ ‭KJV‬‬

Abel was accepted because he was a doer of good so his sacrifice was accepted Cain was not a doer of good so his wasn’t acceptable . God sees things beforehand so Abel the righteous is chosen and Cain the wicked is not .

This shows us how much the flesh matters to God . By the flesh the heir is Cain but by the spirit the heir is the one who does right even being the second born and lesser by the flesh

Esau the first born God knew him beforehand he knew this if esau

“And Jacob sod pottage: and Esau came from the field, and he was faint: And Esau said to Jacob, Feed me, I pray thee, with that same red pottage; for I am faint: therefore was his name called Edom. And Jacob said, Sell me this day thy birthright. And Esau said, Behold, I am at the point to die: and what profit shall this birthright do to me?

And Jacob said, Swear to me this day; and he sware unto him: and he sold his birthright unto Jacob.

Then Jacob gave Esau bread and pottage of lentiles; and he did eat and drink, and rose up, and went his way: thus Esau despised his birthright.”
‭‭Genesis‬ ‭25:29-34‬ ‭KJV‬‬

God chose Jacob because Jacob values Gods blessing and esau sold it for a bowl or pourage stew. God already knew so his blessing comes beforehand to the proper one the second born valued Gods thkngs the first had no value upon them

the thkngs of the flesh are like the first born flesh son and the things of the spirit are like the second born chosen by promise son
