Is fidelity a theological virtue? Is their a difference between the virtue of faith and fidelity?

Yes and no it depends on what context your using it in. if it comes to remaining faithful to a spouse yes it is different than faith as faith is defined as believing in the things not yet seen or trusting in Jesus himself remaining faithful to your spouse is not that.
If you use it in the context of retaining a strict code of conduct and remaining faithful to that which you hold dear yes they are the same thing
both are good virtues however even good things can be taken to far it would be wise to remember that.

Hello and welcome, Will.

I don't expect that you'll get much discussion of "Catholic" virtues per se, because there aren't many Catholics here. There are, however, many Christians.

Faith is the assurance of things not seen (Hebrews 11:1), while fidelity is enduring commitment to a person, entity, or cause. In terms of Christian beliefs, faith (in God and in Jesus Christ) is central. I don't know that I have ever heard fidelity discussed in a Christian context, other than in terms of honouring one's wedding vows.

Personally I think having virtues is good and it is good to ponder what virtues we are to have but if I may ask Will are you Catholic? I only ask because I knew a Catholic person who came here a long time ago and I found his traditions interesting as there is many things about the Catholic faith that escape my understanding.

fidelity:

1a: the quality or state of being faithfulhis fidelity to his wife
b: accuracy in details : EXACTNESSThe movie's director insisted on total fidelity to the book.
2: the degree to which an electronic device (such as a record player, radio, or television) accurately reproduces its effect (such as sound or picture)

faith:

Hebrews, Chapter 11:

1) Now faith is the substance of things hoped for, the evidence of things not seen.

A reasonable person can see the difference between these two IMO....

I strongly suggest you deal with that "UNSURE" answer regarding your Spiritual Status. Do that, begin to study Scripture, and many, many answers will be given to you.
