Ive had nurses visit me but dont recall doctors. Maybe one. All the time I was sick if I had no means of transport to the hospital or clinic I would have just not gone.
One time I fell sick, and the ambulance got called and came and took me to the hospital, and then later they charged me about $70 for the ride. and it was not even that far to the hospital.
 
I thought counsellors just listened and then repeated back everything you said, like an echo. And after they rephrased it into some psych speak then they would ask...and how does that make you feel?
