Yes it is true John Wesley did adopt some of this in his holiness movement.
Sacraments are means to impart grace.

There is a sin not unto death (John 5:16) which is speaking of making mistake (falling short of God's absolute perfection) which is things such as dropping something on the floor, forgetting to do something, or other honest mistakes.
There's no such thing as standalone spiritual salvation. Salvation encompasses body, soul and spirit, ie, the whole person. There will not be salvation of any part if the whole is not saved.
I too believe salvation can't be undone, if we guard what has been entrusted to our care.
Saved is saved, justified is justified and one is positionally sanctified.
God adopts and chastens his children.
